The medallion goes on the ceiling first then the lighting fixture or fan is installed. Generally the hole in the medallion only needs to be wide enough to pass the electrical wires through. After the medallion is secured to the ceiling and the fixture installed the canopy is then attached to cover the hole in the medallion.

Some medallions come in a 2 piece version for those that do not wish to totally take down the light fixture or fan. Just loosen and release the canopy and install a 2-piece medallion. Then reinstall the canopy on top of the medallion.
